Röntgenläkarna väljer ofta att byta undersökningsteknik för att fastställa
misstänkt patologi som inte går att diagnostisera med en metod. Vid
röntgenundersökningar av sinus (bihålor) så kan läkaren välja mellan
konventionell röntgen eller en datortomografiundersökning. Skälet är att
diagnoser som tumör, benpåverkan eller ansiktstrauma inte avbildas lika bra
med den konventionella metoden.
Syftet med den här studien är att jämföra datotomografi och konventionell
röntgen vid sinusundersökningar, vilka för och nackdelar som de olika
metoderna har, hur bildinformationen och diagnostiken skiljer sig, vilka
risker som finns med joniserad strålning samt vilka biologiska effekter
strålningen har på människan. Intervju har gjorts med odontologi läkare som
berättat om för och nackdelar med de olika metoderna, och litteratur och
artiklar har gett kunskap för att komma fram till resultatet. Den stora
användningen av datortomografi gör att det numera inte görs många
sinusundersökningar med konventionell röntgen. I en del av studien har
tidmätningar utförts på datortomografi
sinusundersökningar och även granskningstid, den tid det tar för läkaren
att bedöma bilderna. Det har inte utförts några tidmätningar på den
konventionella metoden på grund av otillräcklig tidsperiod, däremot har det
undersökts för hur lång tid som patienten är bokad för en konventionell
sinusundersökning. Stråldosmätningar har inte heller utförts på någon av
metoderna utan stråldoser för DT och slätröntgen är hämtad ur litteratur
och artiklar.
Det har framkommit ganska tydligt i studien att
datortomografiundersökningar av sinus har flest fördelar och få nackdelar,
den ökade stråldosen som patienterna utsätts för med datortomografi är
marginell enligt odontologi läkaren och dessutom behöver inte patienten
vara med om två undersökningar eftersom datortomografi metoden ger all
nödvändig information bildmässigt.
Abstract in English
When it comes to discover diagnostic pathology in radiology examinations,
the radiologist often choose a different radiology examinations of a
patient to discover a certain pathology, for example if a the radiologist
suspect a tumour in the paranasual area, he would choose to exam the
patient with computed tomography. Because the CT method has several
advantages in discover tumour, destruction or traumatic fractures in the
skull. Pathology such as tumour, destruction or trauma fractures does not
have the same diagnostic image quality in the conventional radiology
method.
The purpose with this study is to compare computed tomography with
conventional radiology in examinations of the sinus, advantages and
disadvantages of the methods, also the image quality and diagnostic quality
is compared. Both methods also expose the patient with ionised radiation:
this study also shows how the damage in the human cell can cause mutations
and cancer. Also interview with a Deontologist has been a source to
information: the interview story told me that the conventional radiology
method for sinus examinations is uncommon these days, only the emergency
radiograph carry out examinations for sinus, the main part of every sinus
examinations is performed by computed tomography. I also have search the
knowledge for the result through literature and articles. A part of this
study is also about time measurements in computed tomography examinations
of sinus, in the conventional method the time has been noted how long the
patient is reserved for the examination, the diagnostic review time has
also been noted, all measurement has been perform in the Sunderby hospital.
This study doesn't have any radiation test of each method, radiation doses
for each method has been collected from literature and articles.
